Apache Server using Worker MPM is freezing and I have to reload

Hello,
I have this weird situation.
I have a simple php application with MSSQL all installed on one server.
For about 23 hours a day it runs very smooth on the default settings.
All of a sudden, all the workers gets busy and not a single one gets idle to serve new requests.
Tried to tweak some settings but that also gets occupied.
What would you suggest to start taking a look? I have asked someone to take a look over the DB for tweaking and indexing stuff, but would that help?
I'm attaching a chart for the workers when the problem happened.
