how to add "guest" OS ?

I have this running on one of the multi boot operating system.
VirtualBox Graphical User Interface Version 6.1.16_Ubuntu r140961
Somehow I managed to add "extension" - download from Internet , however I cannot figure outhow to add "guest OS" using the above.
None of the tutorials I have read so far are very intuitive.
Actually quite outdated.
Even not sure if I can use my USB ISO with Ubuntu OS - no optical disks here.
