Which terminal emulator can do activity alert notifications?

I'm looking for a terminal emulator or terminal mulitplexer which can fire up an alert of some kind, preferably a custom script, when activity resumes after a quiet period regardless of the focus of the terminal. This needs to take place whether the terminal window has focus or not.
I'm not looking for something that rings the bell each and every time there is a new line, but simply does it once at the beginning of activity when new typing appears after there has been a period of quiet.
Where should I be looking? I've looked at quite a few terminal emulators but not seeing what I am looking for. Perhaps that functionality exists but I am not knowing what to look for.
I've also looked at alert-activity, alert-bell, monitor-activity, and monitor-bell in tmux. However, a monitor-activity hook does not trigger if the window being monitored is being used.
