There were gross" failings in the care of a student who died after after suffering a dislocated hip in a rugby accident and developing sepsis in hospital, a coroner has said.
Staff at Southmead hospital in Bristol failed to carry out all the observations needed to keep 20-year-old Maddy Lawrence safe, and failed to screen her for sepsis.
