Christian McCaffrey injury: Status of 49ers RB for Thursday's opener and what it could mean for fantasy football

I feel like I spend half of my hours dreaming about the San Francisco offense these days. Of course, I spend the other half sweating in a panic.

This is life with fantasy football's most boom/bust depth chart. And we have plenty to be nervous about as the 49ers get ready for their Thursday night opener against the Rams, coming live from Australia, with several stars on the injury report.

The perks with San Francisco are obvious. Kyle Shanahan is a brilliant coach, Brock Purdy a plus quarterback and Christian McCaffrey, George Kittle and Mike Evans all look like Hall of Famers. Of course those last three vets are all back-nine players, too - McCaffrey enters his age-30 season, while it's an age-33 lap for Kittle and Evans.

Injury concerns come standard with all three. Kittle tore his Achilles in January. Evans was a limited player all summer, managing a groin injury. It's possible Kittle will have a slow open to the season, and Evans also has to be handled carefully, though I suspect he'll at least repay this summer draft cost.

The polarizing theme is more critical with McCaffrey. He's a legitimate first-round pick, but he's shown an absurd level of boom and bust throughout his career. Consider his career fantasy scan, using the end-of-year rankings from Pro Football Reference (standard scoring).

- 2017: RB15, full season

- 2019: RB1, full season

- 2020: RB51, played three games

- 2024: RB73, played four games

- 2025: RB2, full season

That's quite the stew, isn't it? In five different years, McCaffrey was just about as good as any player on the fantasy field. But in three other seasons, he torpedoed your roster, basically because of injuries. We also should note that his efficiency dropped last year, though elite usage made up for it.

Okay, football is a risky game, and anyone can get hurt - especially running backs. That doesn't qualify as breaking news. But the McCaffrey risk always feels a little different because of his history, his coach and even his time zone.

Fantasy players still feel the sting of the 2024 opener. McCaffrey dealt with calf and Achilles injuries that summer, but it appeared likely he would play in the Monday night opener against the Jets. Instead, McCaffrey was a surprise game-day scratch, while understudy Jordan Mason rolled for 147 yards and a touchdown. Soon after that, McCaffrey was placed on injured reserve, and he didn't make his seasonal debut until Week 9. McCaffrey ended that year with just four starts, 202 rushing yards and zero touchdowns - a crushing blow to fantasy managers.

No NFL coach is likely to be overly forthcoming with injuries and usage plans, but Shanahan has always been especially buttoned-up in this regard. Mason seemed to suggest after the 2024 opener that he knew all along he would start - essentially disclosing the Niners were intentionally misleading everyone on McCaffrey's true status - but later walked those comments back. Reading the tea leaves, I'm inclined to believe Mason's on-the-spot comments to be true (albeit a breach of team confidentiality) and everything said after that night feels like spin and damage control. You can decide for yourself.

Although the Niners play in the NFL's second game this week, they will usually be in the back end of the NFL schedule. That's life as a team on the West Coast. San Francisco has just three games on a Sunday at 1 p.m. ET, the early window (the Niners also have a Thursday game in December). So if this offense is ever tied to a game-time decision player, you might have to make your fantasy decision with incomplete information.

No matter if you roster McCaffrey or not, you have to scout RB Kaelon Black with interest. Black was San Francisco's third-round pick in April and had a solid camp, presumably rising to the No. 2 spot on the team's death chart. But if McCaffrey ever misses time, we don't know for certain that Black would morph into a starter, let along a bell cow. Jordan James could also be in the mix. And sometimes teams tap into surprise candidates when injuries pop up, especially at running back.

Bottom line, Black makes for an interesting stash, currently rostered in 23% of Yahoo leagues. And we'll have our ears to the ground in San Francisco all season, same as it ever was. If you're drafting one more team before the season starts, nod to the fantasy gods as you consider that McCaffrey pick.
