unable to logon, why libvert qmenu showing

don't know at all why this libvert qmenu prompt is showing up. I have no idea what passwod it's expecting. I've tried to reset my user passwrd from th command line, doesn't help.

is there a default password for libvert qmenu? I really need to get into my system.
