Traffic forwarding software?

Afternoon Gents,
I was looking for your help if possible!
I believe there is software available to forward traffic once it hits to a linux server to a different IP address. For instance
Traffic wants to visit 45.65.78.599 PORT 99 but it is blocked by an ISP or local network.
Instead I give the IP address out of 45.943.42.24 PORT 99, this is my linux server.
When the traffic hits 45.943.42.24 PORT 99 it diverts/routes the traffic to 45.65.78.599 PORT 99 hence becoming untraceable. I suppose this is a sort of proxy setup or forwarding? I'm useless at Linux but any help would be appreciated!
