CentOS6 to CentOS7 Migration Error - "fsck error 2"

I am migration a VM from CentOS6 to CentOS7.

I am following below steps :

Ran command #redhat-upgrade-tool-cli --force --iso=<path-to-iso>

After this it asks for reboot and in VM console I can see that upgrade process is completed.

After Reboot I can see CentOS7 in GRUB. After loading GRUB I get below error (Screenshot -> https://ibb.co/sQyZdSn)

After some time I get login prompt but login doesn't work. It gives login prompt again.

I tried to go into rescue mode and troubleshoot but it gives IO error with blue screen. Even rescue mode is not working. ( Screenshot -> https://ibb.co/PrDr13r )
