Disk I/O slowness

Hello All,
I would like to understand more on troubleshooting disk IO issues.
I ran iostat -x command.. observered the iowait, %util, rawait, wawait, svctm...
i can confirm whether there is slowness in disk IO or not.
But still i have some questions..
1) what is causing this disk IO?
is it due to workload or some issue with the disk.
2) which process or files are causing the disk IO slowness..
Can someone help me with the above questions
That will really help me to understand and work on the IO related issues.
